Identify the relationship, name, and e-mail address for each person that will take your 360 survey. Please submit your raters by the above deadline. You can still edit your rater list after you click "submit" by using the link in the email you were sent.

We recommend that you select approximately 7-12 raters. However, there is no set amount of raters required. You need to have at least 2 raters in each category, except self and supervisor, in order to receive a feedback summary for that category.
